How do you make a referral? Just place one call to Team Infusion and we will do the rest.
Please supply the following information when making a referral:
- Patient name, address, phone number, date of birth, height, weight, and any allergies.
- Diagnosis related to the infusion therapy, prescription for the drug, and start date.
- Insurance information with subscriber name and policy numbers (phone numbers if available).
- IV access type - peripheral, PICC, or other.
- Ordering physician information.
